The GTX-680 is actually the last Nvidia model that is potentially possible to flash yourself to have Mac firmware. This is done using tools in Windows, I did this myself with a GTX-680 using Windows via Boot Camp on the Mac Pro itself. Once this is done it will then show the pre-boot options like a real Mac card. Currently both Newegg and Amazon are out of stock for the EVGA Mac Edition GTX 680. A Reddit user claims that a simple BIOS flash of the GeForce GTX 680 could turn it into a GeForce GTX 770. The BIOS ROM image, which probably works with reference-design GTX 680 boards was posted, along with a GPU-Z screenshot of a 'GeForce GTX 770' obtained this way.
There used to be a Mac edition of the GTX-680 and as far as I recall this was only ever sold under the EVGA brand. It is possible to 'flash' the firmware of PC versions of this card to in theory convert them to Mac edition cards I have even done this myself although I do not have or use Adobe CS.
To confirm your card has some form of Mac firmware does it show the standard Apple logo in the middle of the screen when you boot up? If it is completely black until the login screen appears then it does not have Mac firmware and is likely an unaltered PC edition card if it does show the Apple logo it does have Mac firmware.
Of course even if it has been flashed it might have been done badly and be causing your issues.
Something that is not clear from your post is whether you are using the built-in Apple drivers for this card - Apple do provide drivers for the GTX-680 or whether you have downloaded the Nvidia 'web edition' drivers which Nvidia also offer. It would be worth trying the opposite to whichever you are currently using.
